See Your Doctor

Schedule Appointment with your primary care physician/health provider for your baseline physical assessment (know your numbers – glucose, cholesterol, blood pressure, Body Mass Index, etc.)

Step 2 - Gather Your Tools

Get your road map to your Best Self book

Use this book to track your progress. 

Social Determinants of Health Assessment

Gather identified needs from your health risk assessment, social determinants of health and behavioral health assessments and insert in your Best Self book.

Use Your Support

Identify your care team partners (health care provider, family member, friend, agencies, co-worker, etc.) to assist you in your walk to better health.

Step 3 - Take Action

Make A Plan

Immediately after step 2 begin planning your action steps and goals (short and long-term goals) to better health and share with your care team members.

Buddy Up

Identify and Partner with a friend, family member, associate for accountability on you action plan

Stay Up To Date

Track your progress (daily, weekly, or bi-weekly)
Stay focused, Stay connected to your care team and Stay committed to living your best life in better health.

Step 4 – Celebrate Your Successes

Socialize

Success is measured by you. As you reach your short term and long-term goals celebrate your successes along your journey. Post your successes to our community website so we all can celebrate with you.

Brag

Gather your badges of honor along the way and post on your social media profile.

Join Events

Join us for our “It Starts with Me” online celebration events where we highlight participants who have demonstrated a strong personal commitment to this campaign and to living their best health.
